loci and Qloo: the solution

loci is the way home after hospital, built for both sides of care. Clinicians write the plan and track it from their own workspace. Patients and families receive it with a code and follow it step by step at home. The loop is plan, follow, track, adjust, and Qloo's taste AI fills the days in between with comfort that fits the person.

Live: https://app.locicare.xyz (Try the demo: "See it as a clinician" or "See it as a patient", same demo patient on both sides, no sign-up)

The loop

  1. Plan (clinician). In the clinician workspace, start from one of twelve specialty templates, a stroke rehab or orthopedic physio structure template, or the team's own saved templates. Write each medicine schedule, care task, exercise and appointment, plus a plain "why" and a step-by-step "how", general instructions, warning signs and check-in thresholds. Preview the patient view.
  2. Send. One click gives an 8-character code and a private link. The patient types the code in the app or opens the link.
  3. Follow (patient). The plan appears as numbered steps in the clinician's words. Tick-offs, a two-minute daily check-in, an "ask my care team" thread, the when-to-call panel, the journey, the care circle and the Qloo comfort layer.
  4. Track (clinician). Live adherence per item, check-in trends, flagged check-ins with "Mark reviewed", notes and questions, care circle status, an inbox for questions.
  5. Adjust (clinician). Edit the plan in place. The patient sees "Your care team updated your plan" with exactly what changed.

For the person and their family (free)

  • Redeem a code from the care team, or start a journey yourself and copy the discharge letter.
  • The plan, step by step. What to do, why your team asked, how to do it, and when, in the team's words.
  • Ask your care team. A simple thread for practical questions; clearly not for emergencies.
  • Onboarding by specialty. Twelve templates: maternity, surgery, orthopedics, cardiac, neuro rehab, pediatrics, mental health, oncology, respiratory, diabetes, elder care and general. Patients or caregivers can lead.
  • Today. Medicines, care tasks and exercises at the times the team set, one tap to tick off. Next appointment and next milestone.
  • Daily check-in. Mood, pain, sleep, appetite, energy and the plan's warning signs in two minutes. Streaks and badges reward showing up, never health values.
  • When to call. If an answer matches a clinician's rule, loci repeats the clinician's words and shows call buttons for the team and the local emergency number.
  • The road home. An animated road from hospital to home with milestones, appointments and two to four weeks of trends.
  • Care circle. Invite by private link with roles. Shared errands that people claim and finish.
  • Comfort, by Qloo. A soundtrack, shows, reading, meals near home within ticked filters (vegetarian, halal, delivery, soups, and more), gentle places nearby, and a good-day plan that weaves these around the plan steps.
  • Welcome-home card. A shareable card with an arrival soundtrack, first meal and first night in. No health details.

For clinicians and care teams (planned pricing)

  • Own sign-up and workspace at /clinic, with colleagues in a care team.
  • Patient list with status (draft, sent and waiting, following), flags and open questions, sorted by what needs you.
  • Plan builder from specialty templates with why and how per item, general instructions, warning signs and thresholds; patient-view preview; send with a code.
  • Per-patient tracking: today's tick-offs, 7-day adherence per item, five trends, flagged check-ins with review notes, notes and questions, circle status, live activity.
  • Message thread per patient and an inbox of open questions. Update the plan after sending; the patient is told.

How Qloo makes it personal

loci asks for a few favorites (an artist, a show, a book, a podcast, a game) and a home city. Qloo's Insights API returns:

  • artists, shows, films, books and podcasts with high aggregate affinity to those favorites, with explainability so each pick says "Because of Norah Jones"
  • restaurants near home that match all ticked tags, within a walkable or city radius
  • cuisines the taste leans toward
  • parks, gardens, museums, libraries and cafes nearby, optionally step-free or quiet

These become a day: wake up slow with one artist, lunch from a place that delivers, a quiet hour with a book, an evening with a show, wind down with another artist, with the medicine and exercise slots exactly where the team put them.

Safety by design

  • loci invents no medical content. Templates hold structure only.
  • Flags are simple, visible rules written by clinicians.
  • Qloo gets favorites, a city point and generic tags only. Never names, conditions or check-ins. Taste is never used to infer health or eligibility, and picks are labeled aggregate affinity.
  • Consent switches, a consent log, export and one-step delete.

Impact

  • Fewer missed doses and exercises because the plan is visible at the right times to everyone helping.
  • Earlier calls because warning signs are in front of people daily, in the team's own words.
  • Less isolation and better mood because recovery days are built around what the person loves.
  • Clinics get a low-cost, consent-based signal between visits for every specialty.
